At Eye to Eye Optometry, we desire to care for you by filling your insurance.  Below is a list of insurances in which we have a licensing agreement.  If you do not see your particular insurance, please call us at 302-678-EYE2 (3932), and we will be glad to answer any questions you might have.

Blue Cross/Blue Shield: Please bring your identification card.  For some procedures, you may need to get a referral from your primary care physician.

Medicare: Please bring your Medicare identification card. Medicare does not cover some procedures such as the refraction or a bandage contact lens.  If you want or need these procedures, you will be asked to read and sign an Advance Beneficiary Notice as required by Medicare.

Aetna: Please bring your identification card.

Cigna: Please bring your identification card.

Tricare Prime: Please bring your current millitary identification card.

Medicaid(EDS, Diamond State Partners, Delaware Physicians Care): Please bring your current Medicaid identification card.

Coventry Health Care: Please bring your identification card.

United Health Care: Please bring your identification card.

EyeMed: Please bring your identification card.

Vision Service Plan (VSP): We are able to obtain electronic authorizations. The staff will need the policy holder's last four digits of the social security number and date of birth.

Vision Benefits of America (VBA):  The staff will need the policy holder's last four digits of the social security number and date of birth.
